How directory information is sourced, why a profile may exist before a practice has claimed it, and how to correct or remove a listing.
Where the information comes from
- Ontario Ministry of Health — Service Provider Locations
- The provincial dataset of health service provider locations, used as the base record for facility names, addresses and service types.
- data.ontario.ca
- Ontario's open data catalogue, including health facility, long-term care and community service datasets.
- ontario.ca
- Programme and service pages published by the Government of Ontario, used to confirm how a service is classified.
- halton.ca
- Regional Municipality of Halton publications covering local health, senior and community services in Oakville, Burlington, Milton and Halton Hills.
- Public business directories and practice websites
- Publicly published business listings and the practice's own website, used for contact details, hours and services.

Profiles are created from public information
Most profiles on Health Connect Centre were created from public business information — the sources above — and have not been claimed by the practice. An unclaimed profile is labelled as such. Nothing is described as verified unless it has been checked, and paying for a membership never changes verification status. Profile information should be confirmed directly with the practice before you attend.
This directory holds business information only. It contains no patient records, no appointment data and no personal health information, and it never will.
Claiming, correcting or removing a listing
- 1. Claim it. Claiming is free and lets you edit your own details directly. Claim your profile.
- 2. Request a correction. Use the form on this page. Anyone can submit one — you do not need an account. A person reviews every request.
- 3. Request removal. Choose "Request removal of this listing" in the form. Removal is actioned for the practice or an authorised representative; we may ask you to confirm your connection to the practice first.
- 4. Privacy requests. Questions about personal information can be sent to privacy@healthconnectcentre.ca.
